Savannah, GA – Judge Realty recently announced a partnership with Hospice Savannah to offer support and encouragement for aging in place. This partnership involves strategic home modifications aimed at enabling older adults to live independently for as long as possible within the comfort of their homes.
Judge Property Management, a division of Judge Realty, is collaborating with Hospice Savannah’s CAPABLE+ program in this initiative. The CAPABLE+ program is designed to offer strategic support to older adults for independent living, thereby decreasing the need for hospitalization and reducing healthcare costs.
The CAPABLE+ program, run by Hospice Savannah, is geared toward providing support to both low-income adults aged 62 or older and adults aged 55 or older who are willing to pay for the service. The key requirement is that they must be cognitively intact despite encountering some difficulties with self-care activities like bathing, grooming, eating, dressing, or walking.
The program offers a support team consisting of a Registered Nurse, an Occupational Therapist, and a Handyworker who conduct home visits and provide limited care. The team’s main focus is to ensure that older adults can perform daily activities without requiring sustained assistance.
According to the program’s findings, 75 percent of seniors who participated in the CAPABLE+ program demonstrated an improvement in self-care within the span of five months. Furthermore, participants experienced an overall reduction in the number of self-care tasks that required assistance and in symptoms of depression.
Those interested in enrolling in either the CAPABLE program or the CAPABLE+ program in the greater Savannah area can send an email to CAPABLE@hospicesavannah.org. The email should include the client’s name, date of birth, phone number, and street address. A team member from Hospice Savannah will then reach out to conduct an eligibility screening over the phone.
Through this partnership with Hospice Savannah, Judge Realty seeks to provide seniors in the local community with the necessary physical and emotional support they need to live independently. In this endeavor, they are playing a significant role in reducing the physical and mental struggles faced by older adults while also tackling the issue of hospital overcrowding.
